Does Preoperative Biliary Drainage Influence The Outcome Of Resectional Surgery For Ampullary Carcinoma?
A research article published in the World Journal of Gastroenterology addresses this question. This is believed to be the first study to investigate the effect of PBD in ampullary cancer. A team of gastroenterologists led by Dr Steven Joseph Mesenas and hepatobiliary surgeons at Singapore General Hospital divided 82 patients with ampullary cancer planned for surgical resection into those with PBD (n = 35) and those without (n = 47). The authors looked into various outcomes after surgery, such as pancreatic leakage, wound infection, bile leakage, infectious morbidity, intra-abdominal or gastrointestinal bleeding, intra-abdominal abscess, delayed gastric emptying and mortality. They found that the PBD group had a significantly reduced wound infection rate compared to the non-PBD group. More studies should be conducted to assess this benefit, especially in ampullary cancer patients.

Arizona ADAP Cuts Number Of Medications Covered Under Program
The Arizona AIDS Drug Assistance Program (ADAP) has reduced the number of medications it will cover - antiretrovirals and drugs that treat opportunistic infections will not be affected, the Arizona Daily Star reports. The program relies heavily on federal funding. Judy Norton, chief of the state"s Office of HIV, STD and Hepatitis C Services, said the state received $2.3 million less than what it requested from the federal government, requiring the program to make cuts. According to the Daily Star, federal ADAP funds are "drying up as drug costs rise and as more" people living with HIV/AIDS are enrolling in the program. The Arizona ADAP has been serving about 1,100 patients statewide, although the number has gone up in recent months, Laura Oxley, Arizona Department of Health Services spokesperson, said. A letter explaining the changes to the program was sent to clients and providers earlier this week (Innes, Arizona Daily Star, 6/11).

Role Of Overweight Status On Stone-Forming Risk Factors In Children: A Prospective Study

UroToday.com - Gluttony is becoming an ever-increasing problem in our pediatric patients, and this is directly correlated to an increase risk of stone disease. In this prospective study, 44 children who were being followed for obesity in the pediatric department were evaluated with respect to stone disease and stone forming risk factors using a 24-hour urine evaluation. An additional 50 patients who were being seen in the same department for complaints other than stone disease or overweight status were evaluated similarly as the control group. The difference in the demographics of the two groups was their weight and body mass index. However, of most concern, the obese group also had significantly higher systolic blood pressures and were more likely to have established hypertension. Also, in the overweight group the children more consistently demonstrated hypocitraturia and hyperoxaluria compared to their non obese patient counterparts. There was also an increase in urinary calcium excretion in the obese children. Nine percent of the obese children had a history of previous stone disease compared to none in the control group. This study is another in a long list demonstrating the potentially deleterious health effects obesity has on our children globally. The early development of hypertension and stone disease can only have dire consequences for future health issues in these young patients and more attention needs to be directed to this pediatric health crisis.
